چکیده

Methodology Eligible patients have non-resectable liver metastases from histologically confirmed CRC with no or limited extra-hepatic disease and no prior treatment for metastatic disease. Each patient is randomised 1:1 to standard chemotherapy (Oxaliplatin, Folinic Acid, 5-Flurouracil) or chemotherapy plus RE, using minimisation with stratification factors: centre, liver involvement, presence of extra-hepatic disease and intention to treat with a biological agent.

Protocol for Combined Analysis of FOXFIRE, SIRFLOX, and FOXFIRE-Global Randomized Phase III Trials of Chemotherapy +/- Selective Internal Radiation Therapy as First-Line Treatment for Patients With Metastatic Colorectal Cancer

BACKGROUND In colorectal cancer (CRC), unresectable liver metastases are associated with a poor prognosis. The FOXFIRE (an open-label randomized phase III trial of 5-fluorouracil, oxaliplatin, and folinic acid +/- interventional radioembolization as first-line treatment for patients with unresectable liver-only or liver-predominant metastatic colorectal cancer), SIRFLOX (randomized comparative ...

Perioperative FOLFOX4 plus bevacizumab for initially unresectable advanced colorectal cancer (NAVIGATE-CRC-01)

BACKGROUND Perioperative chemotherapy combined with surgery for liver metastases is considered an active strategy in metastatic colorectal cancer (CRC). However, its impact on initially unresectable, previously untreated advanced CRC, regardless of concurrent metastases, remains to be clarified. METHODS A Phase II study was conducted to evaluate the safety and efficacy of perioperative FOLFOX...

First-line selective internal radiotherapy plus chemotherapy versus chemotherapy alone in patients with liver metastases from colorectal cancer (FOXFIRE, SIRFLOX, and FOXFIRE-Global): a combined analysis of three multicentre, randomised, phase 3 trials

BACKGROUND Data suggest selective internal radiotherapy (SIRT) in third-line or subsequent therapy for metastatic colorectal cancer has clinical benefit in patients with colorectal liver metastases with liver-dominant disease after chemotherapy. The FOXFIRE, SIRFLOX, and FOXFIRE-Global randomised studies evaluated the efficacy of combining first-line chemotherapy with SIRT using yttrium-90 resi...

SIRFLOX: Randomized Phase III Trial Comparing First-Line mFOLFOX6 (Plus or Minus Bevacizumab) Versus mFOLFOX6 (Plus or Minus Bevacizumab) Plus Selective Internal Radiation Therapy in Patients With Metastatic Colorectal Cancer

TAKE-AWAY POINTS This multicenter international study randomized patients with metastatic colorectal cancer (mCRC) between first-line systemic chemotherapy alone and first-line systemic chemotherapy combined with yttrium-90 (Y-90) resin microspheres. Eligible patients with nonresectable liver-only or liver-dominant mCRC and no prior systemic chemotherapy were randomized 1:1 between both arms. T...

ACR Appropriateness Criteria: rectal cancer-metastatic disease at presentation.

In 2009, an estimated 40,870 new cases of rectal cancer will be diagnosed in the USA. After decades of treating metastatic colorectal cancer (CRC) with 5-fluorouracil alone, newer agents have resulted in significant improvements in disease-free and overall survival rates. These improvements stem from combinations of newer cytotoxic agents and targeted therapies. Based on performance status and ...
